What I do to fix it is retract one of your operators, rehire an operator on that slot, do 1 exfil streak, then go back to the operator with a broken exfil streak, and it will be working again next time you exfil. 2. The game only "checks" 3 bags on exfil, so don't exfil with more than 3 people on the heli.
