In podiatry, it is a serious dice roll if you are undertaking a fellowship just to beef the resume, network, and hope for a better quality and/or paying job. Like the 7 Habits book says, it always helps to begin with the end in mind; it is a risk to do fellowship just because you reach your last year of residency and don't know what that end is.
